Brandon Woods is a well-established 118-bed Rehabilitation & 24-hour Nursing Care Center with over 50 years of health services to our community and is located less than 2 miles from St. Luke’s Hospital. Accredited by the Joint Commission since 2005, we offer an integrated approach to patient centered care through medical, psychological, social and therapeutic intervention.
Reaching Your Goals
The discharge planning process is initiated during the pre-admission assessment and is the key to your success. Goals are continuously assessed by the interdisciplinary team with input from patients and family members prior to discharge. Home care services are arranged and coordinated with the appropriate agency and all required medical equipment and supplies are delivered to your home to ensure your comfort and safety.
