Chronic Disease Self-Management Program
"My Life, My Health" Chronic Disease State Management Education Program Starting October 9 - November 13, 2009

| Danielle Souza, Executive Director, Brandon Woods Home Care will teach the "My Life, My Health" Chronic Disease Self-Management Program every Friday starting October 9 through November 13, 2009 from 1:00 – 3:30PM at the Mattapoisett Council on Aging. Alongside Danielle co-teaching the workshop is Kim Ferreira. Kim is a Registered Dietician from Coastline Elderly Services. The workshop is given two and a half hours, once a week, for six weeks. People with different chronic health problems are encouraged to attend.
This is an excellent program for caregivers and people with a chronic illness, from asthma to cancer.
Subjects covered include:
1) techniques to deal with problems such as frustration, fatigue, pain and isolation
2) appropriate exercise for maintaining and improving strength, flexibility, and endurance
3) appropriate use of medications
4) communicating effectively with family, friends, and health professionals
5) nutrition
6) how to evaluate new treatments.
Each participant in the workshop receives a copy of the companion book, Living a Healthy Life With Chronic Conditions, and an audio relaxation CD.

May 6, 2009: Danielle Souza and Heidi Roderiques appeared before the Massachusetts Committee on Health Care Financing promoting home usage of TeleHealth Monitoring. Use of TeleHealth Monitoring can reduce hospitalizations by 18-20%.
